Prior to the beginning of the school year, you will receive in the mail a Student and Emergency Contact – Annual Health Information form to be completed and returned to school. It is important that the information is current, filled out completely, signed and returned immediately. It is necessary that we have your contact information and the contact information of three additional friends, neighbors or family members that can be reached in case your student needs emergency medical help. Home, work and cell phone numbers should be included.
Please ensure everyone listed on the form knows that the school may contact him/her and is willing to pick your student up from school when you are unavailable. Notify your school office of any changes or additions that need to be made to your student’s emergency contact information during the school year.
