Step 1. Read Fee Information Sheet
Step 2. Complete the Enrichment/Club Eligibility form online, once per school year. If you have already submitted an Enrichment/Club Eligibility form and/or an Athletic Eligibility form for 2011-12, you do not need to fill out another one. Please go to step 3.
Step 3. Pay fees - You will need to pay the annual $75 participation fee once per school year in addition to the sport activity fee.

Additional Steps 4 & 5 for PERFORMANCE DANCE and MARCHING BAND only:
IMPORTANT INFORMATION REGARDING SPORTS QUALIFYING PHYSICALS: Students participating in Performance Dance and Marching Band are required to have a Sports Qualifying Physical on file with Minnetonka High School. This form is good for THREE YEARS of participation from the date of the exam and must be current through the end of your sport participation season.
If you need a new physical, please print a copy of the PDF form in Step 5 below and have your physician complete, sign and date the form. Be sure to return the signed form to the Activities Office prior to your current physical expiration date in order to be eligible to play.
