| Step 1. Read Fee Information Sheet
Step 2. Fill out and submit on line the MSHSL Athletic Eligibility Statement form including the Parent/Guardian Permit & Health Questionnaire. This form needs to be completed only once per 2011-12 school year, and is NOT the required up-to-date physical form.
Step 3. Fill out Emergency Card online - A new Emergency Card must be completed for EACH SPORT you participate in.
Step 4. Pay fees - You will need to pay the annual $75 participation fee once per school year in addition to the sport activity fee.

IMPORTANT INFORMATION REGARDING SPORTS QUALIFYING PHYSICALS:
All Students participating in MSHSL sanctioned sports are required to have a Sports Qualifying Physical on file with Minnetonka High School. This form is good for THREE YEARS of participation from the date of the exam and must be current through the end of your sport participation season. If you need a new physical, please print a copy of the PDF form in Step 5 below and have your physician complete, sign and date the form. Be sure to return the signed form to the Activities Office prior to your current physical expiration date in order to be eligible to play.

Step 5. Print and fill out a current Sports Qualifying Physical Exam form
Step 6. Full or Partial Scholarship Requests. If you qualify for Free and Reduced Lunch, please print and complete the "Free and Reduced Lunch/Sharing Information" forms and bring in to the Activities Office.

You may NOT begin to register or pay fees on line if you are:
- A transfer student new to the Minnetonka School District entering grades 10, 11, 12 (or, any 9th grade student enrolling after the first day of school)
- A Foreign Exchange student
- Requesting a full or partial scholarship. If you qualify for Free and Reduced Lunch, please print and complete the "Free and Reduced Lunch/Sharing Information" form and bring it to the Activities Office.
If you fall under one of these categories, please come to the Minnetonka High School Activity Office on weekdays between 7:30 a.m. - 4 p.m. to begin your registration process.
