Fees and Registration Forms Procedures

1. All players must sign up online by going to www.minnetonka.k12.mn.us, click on Schools, and choose Middle School West. Then click on the Athletics and Activities Tab, next click on Activity Forms and Fees.
2. Please fill out all of the forms on line and pay online as well.
3. All students participating in MSHSL sanctioned sports are required to have a Sports
   Qualifying Physical on file with Minnetonka Middle School West. This form is good for THREE
   YEARS of participation from the date of the exam and must be current through the end
   of your sport participation season.
4. If you need to check on your student(s) physical date, e-mail your request to:

​Students may not participate in athletic activities until all forms and fees have been completed online.  The two-page transportation form is the only form that needs to be printed and turned into the head coach.

The Middle School Waiver form is only to be completed by students who wish to participate at the high school level.

• Student Athlete checklist: This form needs to be completed and turned into your coach before you can attend practice.
  
  
• Emergency card: A new emergency card needs to be completed for EACH ACTIVITY you participate in.
  
  
• Complete the MMW Athletic Eligibility Form

• Physical Exam: This needs to be on file BEFORE a student can practice or play. A physical is valid for three years from date of issue.
  
  
• Middle School Waiver ONLY for students participating on MHS teams. Please note: Additional fees are applicable for participation in MHS athletics.
  
  
• Transportation forms
  
  
• Pay athletics and activities fees online.


• If financial assistance is needed, please fill out the scholarship form and return to the main office. 
  

1. Visit the Registration Website.
2. Click on ‘Activity Registration Fees’
3. Select MMW
4. Click on the appropriate activity link
5. Run through the checklist – if you already paid the one time $50 annual participation fee, there is an opportunity for you to deduct it from the total.
6. Fill in the details for your child’s Grade (name, address, home phone)
7. Click buy now and follow the on screen instructions – scroll to the RIGHT for the instructions – be sure to identify your student by name so the Activities Coordinator knows who has paid the fees.
